10.5Visa Fraud Monitoring ProgramVisa’s Fraud Monitoring Program flagged the transaction.
11.1Card Recovery BulletinThe merchant apparently did not get authorization for the transaction through an account number listed in the Card Recovery Bulletin.
11.2Declined AuthorizationThe authorization was declined, but the transaction was processed anyway.
11.3No AuthorizationThe transaction was processed without any attempt to gain authorization, or authorization was requested after the fact or for a different amount.
12.1Late PresentmentThe transaction was not sent to Visa within the required time limits.
12.2Incorrect Transaction CodeThe wrong transaction code was used.
12.3Incorrect CurrencyThe merchant made one or more errors related to the transaction currency.
12.4Incorrect Account NumberThe account number in the authorization does not match the account number used in the transaction.

13.2Canceled Recurring TransactionThe cardholder claims to have been charged for a canceled recurring transaction
13.3Not as Described or Defective Merchandise / ServicesThe cardholder claim that the goods or services were defective or not as described prior to the transaction.
13.4Counterfeit MerchandiseThe merchandise was identified as counterfeit by the owner of the intellectual property, a government agency, or a neutral third-party expert.
13.5MisrepresentationThe cardholder claims a purchased item or service was misrepresented and alleges false advertising.
13.6Credit Not ProcessedThe customer claims a credit was due but never processed.
13.7Cancelled Merchandise / ServicesThe cardholder claims this activity was fraudulent, but blames the merchant’s EMV terminal (card-present).
13.8Original Credit Transaction Not AcceptedA voided transaction receipt was not processed (applies to Europe and inter-regional areas only).
13.9Non-Receipt of Cash or Load Transaction ValueCardholder did not receive the full cash withdrawal at an ATM.
4808Warning Bulletin FileIssuer is unable to verify that an authorization code was obtained at the time of the transaction.
4808Authorization-Related ChargebackThe merchant didn’t obtain authorization or got it too late.
4808Account Number Not on FileThe issuing bank can’t find an account with the number used.
4808Required Authorization Not ObtainedThe merchant failed to request necessary authorization.
4808Expired Chargeback Protection PeriodThe transaction was not settled until after its expiration deadline, at which point the cardholder’s account was no longer active/in good standing.
4808Multiple Authorization RequestsA card-not-present transaction was declined by the issuer but later approved using Stand-In processing or X-Code.
4808Cardholder-Activated Terminal (CAT) 3 DeviceUnauthorized transactions processed at a cardholder-activated terminal (CAT).
4834Point of Interaction ErrorA single transaction was processed more than one time.

53Cardholder Dispute of a Recurring TransactionCardholder claims the merchant continued to bill the card after a recurring transaction was cancelled, or without explicit consent from the cardholder.
4853Goods/Services not as Described or DefectiveThe cardholder claims the merchandise received (or service provided) differs greatly from the description provided at the time of purchase.

A01Charge amount exceeds authorization amountThe amount of the authorization approval was less than the amount of the charge submitted by the merchant.
A02No valid authorizationThe charge submitted by the merchant was either declined or the card was expired.

M49Vehicle Rental - Theft or Loss of UseThe cardholder claims to have been incorrectly charged for theft or loss of use of a rental vehicle.
FR2Fraud Full Recourse ProgramAmerican Express Fraud Full Recourse Program flagged the transaction.
FR4Immediate Chargeback ProgramThis is a program that allows American Express to process any dispute immediately as cardholder files a chargeback for any reason than actual fraud.
FR6Partial Immediate Chargeback ProgramThe cardholder has disputed the charge and the merchant has been placed in the Partial Immediate Chargeback Program by American Express.
F10Missing ImprintCardholder did not participate in a purchase and was not provided a copy of the card imprint (card-present environments only).
